 To open a Private Mailbox with Postal Plus Express, the requirements are very specific and not subject to negotiation. The requirements are specified in U.S. Postal Service Regulations and U.S. Federal Law. (Note: Please address your complaints regarding these requirements to your elected representatives not to Team Postal Plus Express.)

All paperwork must be completed at the store and in conjunction with an authorized store employee or requires a Notarized Copy of Identification to be provided.

The following forms are required for each Adult and/or Business Entity that will be receiving mail.

The store has the forms on hand that can be mailed of faxed if you encounter any problems with downloading the forms.
  1. U.S. Postal Service Form 1583 - required and must be signed.
  2. Postal Plus Express Service Agreement.
  3. Postal Plus Express Information Form.
  4. Postal Plus Express Acceptable ID List.
  5. Postal Plus Express Instructions


When opening a box for a business, the USPS Form 1583 requires extra paperwork. Please see a counter agent for assistance.

To complete the USPS Form 1583 for private individuals, you must provide two forms of Identification. The following three (3) items are specifically Not permitted: (1) Birth Certificate (2) Credit Cards and (3) Social Security Cards.

You must provide two forms of ID and at least one must be from group A. The Second may be from Group A or Group B. All ID's must be valid and may not be expired. IDs must be in the name the person opening the box.
